When exporting forms the O3 form build the 'encounter' field is not filled in, it only uses the 'encounterType' field only.
Also the 'published' and 'retired' fields are not present in the export, I suppose that is another bug, but for now it needs to be handled in the initializer. I added a configurable default value for the 'published' field as it might differ between environments.
Btw, this is a fix for issue #328
