Fix Resume and the left column on centered themes

What does this PR do?

Fixes two reader-facing bugs in 1.6.14.

• Resume stays hidden until this visit has a saved heading. The button’s layout rule was overriding the hidden attribute, so Resume showed on every outline.
• Fixed left keeps an 18rem column on themes that center content, including Twenty Twenty-Five. Closing the outline leaves a readable Show outline control.

Show page from 1.6.12–1.6.13 is unchanged.

How was this tested?

WordPress 6.8 and PHP 8.2 in WordPress Playground, Twenty Twenty-Five, as a reader on a published post with the block (fixedLeft, Focus, bookmark, close).

• Wide screen: the outline is 288px, the article is not covered, there is no inner scrollbar, and the page canvas does not shift.
• Resume is hidden until a heading is saved, then sits on the same row as Focus, collapse, and close. Clicking it jumps to that heading and hides again.
• Focus turns the post into a sheet and leaves Show page on screen. Show page restores the outline.
• Close leaves Show outline fully inside the left column. Restore brings the outline back.
• At 800px the outline returns to the top of the content.
• Settings → TOCguide: one section shows at a time, and a type size updates the preview before save.
• No PHP notices on the post or the settings screen. No console errors.
