- `rustc_hir::attrs` re-exports were removed (rust-lang/rust 564f208ba9);
  use the `rustc_attr_ir` crate directly for `LangItem`, `AttrArgs`,
  `AttrItem`, `AttributeKind` and `Attribute`.
- `hir::MutTy` was removed; `TyKind::Ref`/`TyKind::Ptr` now carry the
  type and mutability inline. Mirror this in fhir by removing
  `fhir::MutTy` and making `fhir::TyKind::Ref` carry the type and
  mutability directly.
- `ItemKind::Use` now holds a `UseTree`, and nested imports are no longer
  lowered to separate `use` items. Walk the tree recursively in the
  resolver, accumulating prefix segments so nested globs resolve. Since
  nested paths are relative, rustc no longer strips a trailing `self` in
  `use foo::{self}`; use the last segment of the enclosing prefix as the
  defined name instead.
- Drop the now-stable `unwrap_infallible` feature gate.
- Add tests for deeply nested imports and nested `self` imports.

- `TerminatorKind::Goto` gained a `loop_hint_attrs` field; ignore it when
  lowering.
- `BlobDecoder` gained a required `decode_local_def_id` method. Our encoder
  uses the default `encode_local_def_id`, which writes a full `DefId`, so
  decode it the same way rustc's on-disk cache does: read a `DefId` and
  `expect_local`.
