doc: add CPT-EM (CPT Exact Match) documentation to flow guide

Add comprehensive documentation for the CPT-EM hardware-accelerated
exact match table feature in the flow programming guide. Cover
CPT-EM configuration, lookup architecture, and context cache warming
API. Update existing sections to generalize ACL-specific references,
reflecting support for multiple flow algorithms (ACL, EM, CPT-EM).
