Fix indexed name lookup missing renamed or newly added definitions in existing files. Check indexed file freshness and re-extract only changed files for definition queries; do not rebuild dependency topology. Adds repeated-edit, duplicate-name, TypeScript, Python and Rust coverage. Validation: 25 CLI integration tests passed across index_membership, callers_cli, graph_json and multi_query_cli. This is a targeted correctness fix enabling lazy lookup, not a claim of full incremental graph maintenance or end-to-end session speedup.
